2. Fill Out Your Insurer’s Claim Form:  This form will ask for your personal information, insurance details, and information about the mental health services received. Attach the itemized receipt from your provider to this form. OR upload your receipt to your FSA/HSA provider via their website.
3. Submit the Claim: Send your insurance company the completed claim form and attached itemized receipt via email, mail, or the insurance company’s online portal.
