"An increasing problem of inaccurate diagnoses of adult ADHD has emerged. The potential for misattributing symptoms of ADHD to other mental health disorders, and conversely misattributing other mental health symptoms to ADHD, applies both to members of the public, as well as practitioners who may not have adequate knowledge or expertise in the assessment of ADHD." 

Suggested Guidelines For ADHD Diagnosis:

  • Presenting complaint (s).
  • Full psychiatric history.
  • Review of a non-biased evidence-based FDA approved test.
  • Neurodevelopmental evaluation.
  • Past medical history for potential differentials and contraindications to treatment.
  • Semi-structured interview with open questions and elaboration; not simply “yes or no” questions; with adequate probing during the interview.  Do not simply rely on rating scales or questionnaires.
  • Urine Drug screen and lab work results.   Consider impact of thyroid.
  • All relevant areas of life (e.g., education, employment, leisure, relationships, daily task and health management) need to be reviewed in detail to establish the impact of symptoms and the degree of impairment.
  • Explicit detailing of which DSM-5 symptoms of inattention, hyperactivity and impulsivity are present, pervasive, and persistent; and how they cause at least moderate impairment in at least two domains, with detailed illustrative examples.
  • Discussion of independent evidence (informant-based and other objective data such as school reports or teacher feedback) used to support the diagnosis.
  • Consideration of differential diagnoses & comorbidity (including physical health comorbidity).

Cannabis causes decreased concentration

  •  Decreased concentration is a well-known side effect of frequent marijuana use.
  •  If marijuana is used frequently, it is impossible to tell if your decreased concentration is from marijuana or ADHD.

Lack of ADHD treatment has negative impacts:

  • 33% more ER visits >7 medical visits a year 
  • 2-4x more motor vehicle crashes 
  • 2x risk of substance use disorders & earlier onset 
  • 1.5-3x risk of obesity 
  • 2.8-3.3x more likely to develop type 2 diabetes 
  • 3x greater risk of suicide 
  • More parental divorce or separation 
  • Higher dropout rates & lower earnings 

Assessments that rely on a checklist of closed questions (“check boxes”) based on the 18 ADHD symptoms of inattention, hyperactivity and impulsivity run the risk of confirmation bias, and may fail to properly consider important differential diagnoses.

Evidence-based Guidelines for the Diagnosis of ADHD Do Exist.

Evidence based guidelines for the diagnosis and management of adult ADHD have been developed by the National Institute of Health and Care Excellence (NICE), the British Association for Psychopharmacology, and The Royal College of Psychiatrists in Scotland who sought to promote the highest standards of care for adults with ADHD, and influence practice both nationally and internationally.

Over 60% of those with adult ADHD present with one or more comorbid disorders.   Misdiagnosis, in either direction, can delay the initiation of appropriate pharmacological and/or non-pharmacological interventions
